Resumo
Esta revisão narrativa da literatura investiga a eficácia das abordagens de cuidados centrados na relação, particularmente a metodologia Humanitude, na redução da recusa de cuidados em pessoas que vivem com demência (PQVD). Com a recusa de cuidados a afetar até 60% das PQVD, este fenómeno apresenta desafios significativos à prestação de cuidados de saúde, à qualidade de vida das pessoas que recebem cuidados e ao bem-estar dos cuidadores. O estudo sintetiza a evidência atual sobre a prevalência, causas e consequências da recusa de cuidados; examina as abordagens de cuidados centrados na relação em enfermagem; e explora como as técnicas da Humanitude podem contribuir para a redução da recusa de cuidados.
Os resultados indicam que os cuidados centrados na relação, especialmente o foco da Humanitude em profssionalizar o olhar, palavra, toque e verticalidade, podem melhorar significativamente a qualidade dos cuidados, reduzir os sintomas neuropsiquiátricos e o esgotamento dos cuidadores. Os estudos relatam uma diminuição dos incidentes de recusa de cuidados, uma melhoria da cooperação durante as atividades de cuidados e um aumento da satisfação dos cuidadores após a implementação da Humanitude.
Contudo, a integração bem-sucedida destas abordagens requer a superação de desafios como mudanças na cultura organizacional. A revisão enfatiza a necessidade de programas de formação abrangentes, apoio contínuo e integração nos currículos de educação em enfermagem.
Embora promissora, a investigação atual é limitada por estudos de pequena escala e métodos de implementação heterogéneos. A investigação futura deve centrar-se em estudos longitudinais de larga escala em diversos contextos de cuidados de saúde para estabelecer a eficácia a longo prazo e a relação custo-eficácia.
À medida que a prevalência global da demência aumenta, a incorporação de abordagens centradas na relação, como a Humanitude, na prática e educação de enfermagem poderá revolucionar a qualidade dos cuidados para as PQVD e melhorar significativamente o bem-estar dos profissionais de saúde.
